RMA System Explanation

Phoenix Medical Devices utilizes a Return Material Authorization (RMA) process to coordinate the return of a product for any reason.

Download our RMA Request Form. Print, complete and mail or fax the form to our office – ATTN: Customer Service. Fax number and mailing information is included on the form. Do not send your return package to us yet.

Upon receipt we will send you, via fax, your RMA number. We request that you prominently print this number on the return package. This will expedite the processing of your return through our system.

If you have difficulty downloading the RMA form or if you do not have convenient access to a fax machine or email service please call our Customer Service Department at 800-689-9892 and we will process your RMA paperwork over the phone.
